I just noticed the photos added to the op. I realize that photos can be quite deceiving at times. But I see absolutely nothing in any of those photos that indicates he needs to gain any weight at all. In fact (again based upon those photos), I would be quite wary of him gaining weight and be making an effort to keep him right where he's at or slightly below.

He was really ribby. That has filled out, but his hips are still sunken in. Thats what I am mainly worried about.
