That is strange, maybe he did try to lay down. Usually, the reason why horses fall in slant loads is because 1) they are tied to long, or 2) there isn't any or enough sawdust.
Instead of using the tie hooks in the trailer to tie to, I would start using twine. You just make a loop tied to the trailer hook and tie your lead rope to that. That way, if they do fall or get into a prediciment like that, their own weight will break the twine. It's cheap, but it works. We had a horse that went down in the trailer, and the breaking twine probably saved his life.

Wow, talk about an exciting day! Congrats on the job well done - sorry to hear about Smokey going down in the trailer. I'm relieved to hear he's ok. That would have rattled anyone. My gut dropped when I read that line.
